KEY FOB DISTRIBUTION POLICY AND AGREEMENT

This agreement applies to all buildings on the premises of the Islamic Center of Johnson County.

Phone numbers may be used for emergency contact or messages.
Email will be added to ICJC group email so you are able to receive alerts and announcements.

By filling out this form, I agree to adhere to the following rules set forth by the ICJC:
1. The key is not transferable and is the property of ICJC.
2. I will pay a non-refundable fee of $10/year for service fee per key
3. I am responsible for the key and its use.
4. I will be vigilant upon entering the premises and will only allow people I know into the building.
5. I will not prop open any doors and ensure the door shuts behind me upon entrance or exit.
6. I will do my best to ensure the building is secure as I depart the building.
7. I will notify the Board of Trustees or Facilities Manager immediately if my key(s) is lost or stolen.
8. I will surrender my key(s) to ICJC upon request or when it is no longer needed
9. Management can program the keys they consider fit for the security of the premises
10. Whoever enters the masjid should scan their fob for their safety.
11. Keys will only be issued to Muslims living in the Kansas City Metro or Johnson County. City must be listed on Drivers License.
