AGREEMENT

By agreeing to this media release form, you agree to be bound by, and to comply with, these Terms and Conditions. If you do not agree to these Terms and Conditions, please do not use tick the box.

PLEASE NOTE: We reserve the right, at our sole discretion, to change, modify or otherwise alter these Terms and Conditions at any time. Unless otherwise indicated, amendments will become effective immediately. Please review these Terms and Conditions periodically.

Media Authorization and Release
I hereby grant permission to the Islamic Center of Johnson County (ICJC), including its staff, volunteers, affiliates, and contracted personnel, to photograph, record, and otherwise capture my image, likeness, voice, or statements (or those of my minor child, if applicable) during participation in ICJC-sponsored events, programs, or activities.

I authorize ICJC to use, reproduce, modify, publish, and distribute such media in all forms and formats—including but not limited to printed materials, digital publications, websites, and social media platforms—for the purpose of marketing, promotion, public relations, and documentation.

I understand that ICJC may share this content with trusted personnel and third parties who assist in curating, managing, or distributing content on behalf of the organization.

Waiver of Liability and Rights
I understand and agree that:

I will receive no compensation for the use of my (or my child’s) image or voice.

All media content becomes the property of ICJC and may be used in perpetuity unless otherwise requested in writing.

Any request for removal of media content must be made in writing to ICJC. Due to the nature of digital content distribution, I acknowledge that such requests may take up to two (2) years to fully process and remove from all platforms and partner materials, where technically feasible.

I hereby release and hold harmless ICJC, its leadership, employees, volunteers, affiliates, and contractors from any and all claims, demands, or causes of action arising out of the use of such media.
